Generic Abiraterone

Common use

Abiraterone is an oral medication used in the treatment of prostate cancer, particularly metastatic castration-resistant prostate cancer (mCRPC). It works by inhibiting an enzyme (CYP17) involved in androgen production, reducing levels of testosterone that fuel prostate cancer growth. Abiraterone is usually prescribed in combination with corticosteroids like prednisone. It helps slow disease progression and improve survival outcomes in advanced cases.

Dosage and direction

Abiraterone is typically taken once daily on an empty stomach—at least one hour before or two hours after eating. It must be taken with prednisone to reduce the risk of side effects associated with low cortisol. The dosage may vary depending on individual patient needs and response to treatment. Always follow your oncologist’s instructions exactly and do not alter the dose without consulting your doctor.

Precautions

Before starting abiraterone, inform your doctor of any history of heart disease, liver problems, or hypertension. Because the medication can affect electrolyte balance and liver function, regular monitoring of blood pressure, liver enzymes, and potassium levels is necessary. Women, especially those who are pregnant or may become pregnant, should avoid handling crushed or broken tablets due to risk of harm to a developing fetus. Patients should also maintain adequate hydration and follow a low-sodium, low-fat diet if advised.

Contraindications

Abiraterone is contraindicated in individuals with severe hepatic impairment and those allergic to abiraterone acetate or any excipients. It is not approved for use in women or children. Concomitant use with drugs that strongly induce CYP3A4 enzymes should be avoided. The drug should not be taken with food, as this increases systemic exposure and risk of toxicity.

Possible side effect

Common side effects include joint pain, fluid retention, high blood pressure, and low blood potassium levels. Liver function abnormalities are also frequently observed and require periodic lab monitoring. Fatigue, hot flashes, and urinary tract infections may occur. Serious but less common effects include adrenal insufficiency, arrhythmias, and liver toxicity.

Drug interaction

Abiraterone may interact with drugs metabolized by CYP3A4, CYP2D6, and CYP2C8 enzymes, potentially altering their effectiveness or toxicity. Caution is needed when co-administered with medications like warfarin, certain antidepressants, or antiepileptics. Strong CYP3A4 inducers (e.g., rifampin, phenytoin) can reduce abiraterone levels and effectiveness. Always provide your doctor with a full list of medications and supplements you are taking.

Missed dose

If you miss a dose of abiraterone, take it as soon as you remember that day. If a whole day has passed, skip the missed dose and take your regular dose the next day. Do not take extra tablets to make up for a missed dose. Maintain a consistent daily schedule to ensure effectiveness.

Overdose

Overdose may lead to symptoms such as high blood pressure, low potassium levels, or liver toxicity. In case of suspected overdose, seek immediate medical attention. There is no specific antidote; treatment is supportive based on symptoms. Do not exceed the prescribed dosage under any circumstances.

Storage

Store abiraterone at room temperature, away from moisture and heat. Keep the tablets in the original container, tightly closed, and out of the reach of children. Do not use after the expiration date. Dispose of unused medicine as instructed by your pharmacist or healthcare provider.
